Insurance
We bill your insurance as a courtesy to you, any unpaid balances or delays in insurance payments are your responsibility.
Insurance companies normally pay a percentage of the total fee. Co-payments are due on day of treatment. Payment estimates given to us by your insurance company are no guarantee of payment.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
